CCHN Plans for National Healthcare Decisions Day

As the convener of the Chautauqua Cares Coalition (CCC), the Chautauqua County Health Network will be working with members representing hospitals, skilled nursing facilities, Hospice, Office for the Aging among others to coordinate activities for National Healthcare Decisions Day (NHDD) on Wednesday, April 16th to raise awareness around Advance Care Planning.

According to information provided on the official NHDD website, studies indicate that most Americans have not exercised their right to make decisions about their healthcare in the event that they cannot speak for themselves. NHDD along with other local initiatives will help Chautauqua County residents understand that making future healthcare decisions includes much more than deciding what care they would or would not want; it starts with expressing preferences, clarifying values, identifying care preferences and selecting an agent to express healthcare decisions if patients are unable to speak for themselves.

CCC members will be available to speak with regarding Advance Care Planning at Heritage's Chautauqua Mall location on April 16 from 8 a.m.-2 p.m. More activities are in the works, so please check back for more updated information.
