Steps to a HealthierNY funding allowed for collection of Chautauqua County health behavior data called BRFSS from 2004 through 2008. This data provides a local view of resident's access to care, physical activity, nutrition and tobacco use health behaviors. It also provides asthma and diabetes incidence and self-management outcomes from the same time period.
