Resources for Decision Making

Palliative Care Resource Directory Now Available

At the Chautauqua Cares Coalition Annual Meeting, CCHN staff unveiled the latest addition to their end-of-life toolkit- the Palliative Care Resource Directory. This tool can be used to find providers of very specific services throughout Chautauqua County that can ease the burden of an individual or family facing a life-limiting illness. For help using the database or to suggest services or providers, please contact CCHN at 338-0010 or by emailing cchealthnetwork@gmail.com.

 
MOLST Forms Available

The Medical Orders for Life Sustaining Treatment (MOLST) form is a bright pink order signed by a NYS licensed physician that communicates patient wishes to health care providers. These valid medical orders must be followed by all health care professionals in all sites of care, including the community. The form includes medical orders and preferences regarding:

  • CPR
  • Intubation and mechanical ventilation
  • Artificial hydration and nutrition
  • Future hospitalization and transfer
  • Antibiotics

A sample of this form can be downloaded here, but the actual form is available at your doctor's office or by contacting CCHN at 716-338-0010 or emailing seekingpeace@stny.cc.com.

Cara Seekings recently attended the Education on Palliative and End of Life Care (EPEC) Training Conference at Northwestern University where she received certification as an EPEC trainer. She is now available to provide inservice training for local health care provider offices and health care facilities. This training can be provided onsite to meet the specific needs of your organization or staff.

Chautauqua Cares Health Care Proxy Registry

The FREE Chautauqua Cares Health Care Proxy Registry is a 24-hour, 7 days a week access system for medical care providers. Hospitals, physicians, and EMS responders will have immediate access to a copy of your Health Care Proxy should you be unable to communicate. Physicians, rescue workers, or emergency staff can locate and immediately download the form wherever they have access to the Internet.

5 Step Process

  1. This is about what is important to you and what health care wishes you want carried out if you are unable to communicate for yourself.
  2. Appoint a health care agent, a person to speak for you should you be unable to speak for yourself.
  3. Talk to your agent and family about your wishes.
  4. Put your wishes in writing using the NYS Health Care Proxy Form. To download forms from other states, please click here.
  5. If you receive health care services in Chautauqua County, mail a completed copy of your proxy to : CCHN, 200 Harrison Street, 2nd Floor, Jamestown, NY 14701 or fax for FREE entry in the Chautauqua Cares Health Care Proxy Registry.

Along the Way Booklet Now Available

Along the Way is a booklet developed by CCHN in partnership with Office for Aging and Hospice for use in physician offices. Patients receiving a life limiting diagnosis will find the booklet helpful as a discussion tool that helps them outline treatment and personal goals.
