Policy Systems Environmental Change

Chautauqua County Creating Healthy Places to Live, Work, and Play (CHP) is a five year initiative funded by the New York State Department of Health to prevent obesity, type 2 diabetes, and other chronic diseases. Work is underway to address the ongoing need for sustainable policy, environment, and systems changes to support better physical and nutritional health.
Strategies include:
· Creating, rehabilitating, maintaining safe and accessible parks and recreational facilities
· Developing transportation policies and environmental changes to ensure streets are safe, accessible, and convenient for all users of the roadway
· Creating community gardens
· Promoting the use of farmers markets
· Exploring the feasibility of establishing a grocery store in a food desert
Our community partners include schools, municipalities, not-for-profits, and churches leveraging and expanding existing efforts to create supportive environments that will accelerate improvements in individual behaviors and health outcomes throughout the County. The early phase of the project is focused on the East Side neighborhood of the City of Jamestown, where residents are engaging in identifying problems and solutions to active living and healthier eating.
