One of the hallmarks of Creating Healthy Places is the focus on partnership and collaboration and the ongoing process of facilitating policy, systems and environmental changes focused on healthy lifestyles.  These coordinated efforts to transform the community are expected to continue long after the CHP grant funding is expended.

Our community partners include schools, municipalities, not-for-profits, and churches.  We are working diligently to expand existing efforts to create supportive environments that will accelerate improvements in individual behaviors and healthy outcomes throughout the County.
